The wellness questionnaire is a guide to help you evaluate your feelings and negative thought patterns you may be experiencing. It helps you to try and think about these things different and to spot when you might be reacting badly to the situations you are experiencing.
I developed the wellness questionnaire for myself during my recovery. Use it as often or as little as you feel you need to. Now I have learnt how to spot my triggers I don’t need to use it as much, only when I see myself behaving in a destructive manner or when I am having a down day. But it is helpful to use it even when you feel ok because it trains you to spot the signs early on.
